Hospital Discharge Transportation
Total Care Transport provides same-day and short-notice hospital discharge transportation across Southern Ontario. When a patient is cleared to leave hospital, getting home safely and comfortably is the priority. Our trained driver-attendants coordinate directly with hospital discharge teams, come to the patient's room, and deliver safely to the door at home or a receiving facility. Serving Halton, Brampton, Kitchener-Waterloo and Etobicoke, available 24 hours a day, seven days a week.
How Hospital Discharge Transport Works
Discharge timing is rarely predictable. We build our service around that reality.

Call us when discharge is confirmed. As soon as you have a discharge time, call us at 905-878-1170. Same-day bookings are available. Give us the hospital name, floor and room number, destination address, and the patient's mobility needs.

We coordinate with the care team. We work with the nursing team on pickup timing. Discharge can run early or late and we accommodate both.

We come to the room. Your driver-attendant comes directly to the patient's floor. You do not need to arrange transport to a lobby or entrance.

Safe delivery to the destination. We transport the patient home, to a long-term care facility, retirement home or any receiving address. We assist to the door and confirm the patient is safely received before we leave.

If you are a hospital discharge planner, social worker or care coordinator, Total Care Transport is a Mohawk Medbuy approved provider serving hospitals across Ontario. We are experienced with the discharge process and understand the documentation and coordination requirements involved.

Hospital Discharge FAQ
How quickly can you arrange discharge transport?
Same-day bookings are available in most cases. Call us as soon as discharge is confirmed and we will do our best to accommodate your timing. The more notice we have the better, but we understand discharge is often last-minute.
What information do I need when I call?
Hospital name and location, floor and room number, patient name, discharge time if confirmed, destination address, patient mobility needs (can they sit upright or do they need a stretcher), and any equipment requirements such as oxygen or a stair chair.
Can a family member travel with the patient?
Yes. A family member or escort is welcome to ride along at no extra charge. Let us know at booking so we can ensure space.
Do you come to the patient's room or do we need to bring them to the entrance?
We come directly to the patient's floor. Your driver-attendant will check in at the nursing station and come to the room. You do not need to arrange transport within the hospital.
Can you transport a patient who was just discharged from surgery?
Yes. Post-surgical patients who cannot sit upright require stretcher transport. We have two-person stretcher teams available for this. Let us know at booking that the patient is post-surgical and we will assign the appropriate vehicle and crew.
What if the discharge time changes after I book?
Call us as soon as you know. Discharge times shift frequently and we are used to adjusting. We would rather know early than arrive at the wrong time.
Is hospital discharge transport covered by OHIP?
Hospital to home discharge transport is generally not covered by OHIP. Hospital to hospital transfers ordered by a physician are typically covered. Programs that may help with home discharge costs include the iCare Transportation Fund and ODSP for eligible recipients. We provide receipts and documentation for all trips to support any insurance or program claims.
Can you take the patient to a long-term care home or retirement residence instead of home?
Yes. We transport to any receiving address including LTC homes, retirement residences, rehabilitation facilities and private residences. Just provide the destination address when you book.
